The pattern is always the same. You fall asleep without trouble. Then somewhere before dawn you're awake with the duvet off one leg and a damp patch at the small of your back, and it's the third time this week.
Most women file that under insomnia. It usually isn't. It's temperature.
Sleep runs on a temperature curve
Your core body temperature isn't a fixed number. It moves on a daily rhythm, and the most consequential part of that rhythm happens before you're even asleep. Temperature starts falling roughly two hours ahead of sleep onset, bottoms out in the early morning, and climbs again in the hours before you wake.
That fall isn't a side effect of sleep. It's the cue. Research on thermoregulation has found you're most likely to drop into deep non-REM sleep while your temperature is falling fastest, not when it's at its lowest (Frontiers in Neuroscience, 2019). The rate of the drop matters more than the number.
Your body pulls this off by opening blood flow to your hands and feet, which act as radiators and dump heat out through the skin (Physiology and Behavior). It's why cold feet keep you up, and why a warm bath an hour before bed works. You aren't warming yourself. You're giving the heat somewhere to go.
Why you wake up at 3 a.m.
The second half of the night is REM-heavy, and REM is where your thermostat goes offline. Sweating and shivering, the two levers your body pulls to correct its own temperature, are largely suspended while you're in REM (Sleep and Thermoregulation, NIH).
Which means the hours when you have the least ability to fix your own temperature are exactly the hours you're most likely to be woken by it. At 11 p.m. your body can quietly self-correct. By 3 a.m. it has one blunt option left, and that option is to wake you up.
Then add a hot flash on top of it. Polysomnography has tied objectively measured hot flashes directly to nighttime awakenings, and the frequency and severity of vasomotor symptoms track closely with insomnia severity through the menopause transition (Climacteric, 2023; NIH). Peak prevalence in the US runs between 60 and 80 percent, and somewhere between a third and a half of women describe theirs as moderate to severe (NIH).
The variable most women never change
Three things decide how hot you sleep: the room, the bedding, and what's against your skin.
Most women fix the first two. Thermostat down to 65. The heavy duvet gets swapped for something lighter. A cooling mattress topper arrives, helps for a few weeks, then quietly stops helping, because a topper can only pull heat off the parts of you touching it.
The third one sits against you all night and almost never gets touched. Cotton is lovely until it's wet; then it holds the moisture on your skin and goes cold and clammy for hours. Polyester traps heat by design. Both of them work against the drop your brain is waiting for.

Three things to try tonight
Drop the room to 65 before you get into bed, not after. The fall needs a head start.

Change what you're sleeping in. It's the last variable, and for most women it's still working against them.
